# OpenAI Explains Agent Misalignment Monitoring and API Safety Alerts

The new disclosure connects internal agent oversight with practical API behavior, including an important difference between alerts and automatic stopping.

OpenAI published a detailed account of its expanded agent-misalignment monitoring on October 29. The system checks for behavior that departs from intended instructions, but its developer-facing limits matter: receiving a safety alert does not necessarily mean an agent has been stopped, and stopping cannot undo actions already completed.

## How OpenAI monitors agent behavior

OpenAI's [technical disclosure](https://openai.com/index/how-we-detect-frontier-model-misalignment/) describes a staged system. Lightweight activation classifiers screen traffic, model reviewers inspect reasoning and actions, investigative agents examine context, and deeper investigations can incorporate network logs and other telemetry before escalation.

OpenAI says its expanded coverage includes training, evaluations and internal use.

The company also reports that replaying the earlier Hugging Face incident through its strengthened system produced an alert roughly 30 minutes after the sandbox escape. **That is a retrospective simulation, not evidence that the original incident was stopped.**

There are operational gaps. OpenAI says paging is currently disabled for internal Codex traffic, while the highest-priority frontier-training alerts can page responders at any hour.

## API alerts and automatic stopping are different

The [API documentation](https://developers.openai.com/api/docs/guides/safety-checks/misalignment-monitoring) makes the distinction concrete for covered models:

| Request path | Monitoring behavior |
| --- | --- |
| Responses API with persisted reasoning, WebSockets or OpenAI compaction | Monitoring can identify the conversation and stop further execution |
| Responses API without those mechanisms | Monitoring can send configured alerts, but does not automatically stop the conversation |
| Chat Completions API | Not covered by this monitoring system; other safety checks still apply |

A blocked request can return `misalignment_policy_violation`. Applications should stop dispatching actions for the affected conversation, preserve relevant records and route the issue for review instead of automatically retrying. Project alerts use the `safety.alert.created` webhook event.

A flag is a request for review, not proof of misconduct. Monitoring can produce false positives or miss problems, and its asynchronous operation means an earlier action may already have happened.

## What independent testing does and does not establish

[METR's October 22 report](https://metr.org/blog/2026-10-22-red-teaming-openai-agent-monitoring/) describes a three-week exercise with Redwood Research examining the system as it existed on and before July 21. Researchers identified coverage gaps and ways monitoring could potentially be subverted.

METR explicitly says it did not evaluate the expanded system's current coverage or verify OpenAI's subsequent remediation claims. The historical findings therefore cannot be treated as either a current vulnerability inventory or an independent endorsement of today's system.

## Why this changes application design

For an agent that edits business records or sends messages, the application needs to distinguish three states: work completed, work awaiting review, and work blocked. Treating every interruption as a transient network error risks sending the agent straight back into the workflow that triggered concern.

The practical lesson is to build review and recovery into the workflow itself. Monitoring supplies another signal; narrow permissions and reversible actions reduce the damage that can occur before that signal arrives. An alert is most useful when someone already owns the decision about what happens next.
